Save the time and expense of utilizing paper forms that have to be handwritten, faxed and/or mailed.  Let me design forms that you can post on your companies intranet, be filled-in online and mailed electronically.  

Utilizing electronic forms increases productivity and  decreases the risk of errors due to illegible handwriting which can lead to data entry mistakes.In conjunction with the electronic forms I can also design a web-based Forms Index along with instructions for filling out more complex forms.

I have designed and implemented an entire set of Credit Forms and created a related Credit Forms Index including instructions.  Some of these forms included:

Detailed Credit Approval Workbooks for the Commercial and  Industrial and Commercial Real Estate portfolios.  These workbooks included worksheets detailing Debt Analysis, Collateral Summary, Cash Flow Analysis, Real Estate Loan-to-Value, Personal Financial Statements, Income Property Template, Credit Policy Exception Checklist and other related forms.  Each worksheet auto-fills with information from preceding worksheets.

Term Loan Brief Sheets for the various Commercial loan types, i.e. Retail Shopping Centers, Industrial, Nursing Homes, Hotels and others.
